If you are a new client, you have the option of visiting our secure client page (just click the link below) to register as a new client and complete our new client forms. The site is HiPAA compliant and secure. As part of the process, you will sign your consent to treatment, Notice of Privacy Practices, and complete the intake form. These are the same forms you complete when you visit your doctor for the first time. Once you have registered, please call the office at 469.629.8213 to schedule your first appointment.
Current Life by Design Wellness clients may use the secure client area to schedule appointments, pay session invoices, or email your counselor via secure email.
